Claims

Abstract

Methods and apparatus for constructing a form are disclosed. A form includes a substrate a first line of weakness formed on the substrate to separate a central area and a perimeter matrix. The perimeter matrix at least partially surrounds the central area. A fold line formed on the central area to divide the central area into a first side and a second side. A second line of weakness and a third line of weakness are at least partially formed within the central area. The second and third lines of weakness are coupled to the first line of weakness. The second and third lines of weakness form opposing removable portions that are symmetric about the fold line. When the central area is folded about the fold line and the opposing removable portions are removed, the second and third lines of weakness substantially align to create an opening for suspending the form.

What is claimed is: 
     
         1 . A form comprising:
 a substrate having a first face and a second face, the substrate comprising a central area and a perimeter matrix that surrounds the central area;   a first line of weakness formed in the substrate;   a second line of weakness formed in the substrate adjacent to the first line of weakness and coupled to the first line of weakness at a first junction;   a third line of weakness formed in the substrate and coupled to the first line of weakness;   a fourth line of weakness formed in the substrate and coupled to the second line of weakness;   a fifth line of weakness formed in the substrate distal to the first line of weakness;   a sixth line of weakness formed in the substrate distal to the second line of weakness and coupled to the fifth line of weakness at a second junction;   a fold line formed on the substrate between the first junction and the second junction, wherein the first, second, third, fourth, fifth and sixth lines of weakness define the central area and the perimeter matrix and the fold line divides the central area into a first side and a second side; and   a seventh line of weakness and an eighth line of weakness, wherein the seventh and eighth lines of weakness are coupled to the fold line and form a removable portion to be removed to create an opening for suspending the form.   
     
     
         2 . The form of  claim 1 , wherein when the first, second, third, fourth, fifth, and sixth lines of weakness enable the central area to be separated from the perimeter area, and the central area is foldable about the fold line to form a double layered form. 
     
     
         3 . The form of  claim 2 , further comprising:
 an adhesive layer having a first side and a second side, the first side of the adhesive layer coupled to at least a portion of the second face of the substrate; and   a liner covering the second side of the adhesive layer, wherein the liner is removable and the central area is foldable about the fold line, and wherein the adhesive layer is to couple the second face of the first side with the second face of the second side to form the double layered form.   
     
     
         4 . The form of  claim 2 , further comprising a tab coupled to a first face of the first side and a first face of the second side to couple a second face of the first side and a second face of the second side in an opposed configuration. 
     
     
         5 . The form of  claim 2 , wherein when the central area is folded about the fold line, the first line of weakness substantially aligns with the second line of weakness, the third line of weakness substantially aligns with the fourth line of weakness, and the fifth line of weakness substantially aligns with the sixth line of weakness. 
     
     
         6 . The form of  claim 1 , wherein two or more of the first, second, third, fourth, fifth, or sixth lines of weakness are integral. 
     
     
         7 . The form of  claim 1 , wherein the seventh line of weakness and the eighth line of weakness are not directly coupled to any of the first through sixth lines of weakness. 
     
     
         8 . The form of  claim 1 , wherein the two or more of the third, fourth, fifth, sixth, seventh, or eighth lines of weakness are integral. 
     
     
         9 . The form as defined in  claim 1 , wherein the removable portion is in the form of a circle, a diamond, a triangle, a tear drop, a key hole, or a combination thereof 
     
     
         10 . The form of  claim 1 , wherein the first, second, third, fourth, fifth, and sixth lines of weakness are used to separate the central area from the perimeter area, wherein at least an adhesive layer or a tab couples a second face of the first side in an opposed relation to a second face of the second side to form a double layered form, and wherein the seventh and eighth lines of weakness substantially align. 
     
     
         11 . The form of  claim 1 , wherein one or more of the first through eighth lines of weakness include perforations. 
     
     
         12 . The form of  claim 1 , further comprising variable indicia. 
     
     
         13 . The form of  claim 1 , further comprising a security marking. 
     
     
         14 . The form of  claim 13 , wherein the security marking is one or more of a hologram, microprinting, a pantograph, barcode, or color shifting inks. 
     
     
         15 . A form comprising:
 a substrate having a first face and a second face, the substrate comprising:   a first line of weakness formed on the substrate to separate a central area and a perimeter matrix, the perimeter matrix at least partially surrounding the central area;   a fold line formed on the central area to divide the central area into a first side and a second side; and   a second line of weakness and a third line of weakness at least partially formed within the central area, the second and third lines of weakness being coupled to the first line of weakness, the second and third lines of weakness form opposing removable portions that are symmetric about the fold line, wherein, when the central area is folded about the fold line and the opposing removable portions are removed, the second and third lines of weakness substantially align to create an opening for suspending the form.   
     
     
         16 . The form of  claim 15 , wherein the opposing removable portions intersect opposing lateral edges of the central area. 
     
     
         17 . The form of  claim 15 , wherein the openings are symmetric about a lateral axis of the central area. 
     
     
         18 . The form of  claim 17 , wherein the lateral axis is orthogonal to the fold line. 
     
     
         19 . An apparatus comprising:
 a substrate having a first face and a second face, the substrate comprising:
 a first line of weakness formed on the substrate to separate a central area and a perimeter matrix, the perimeter matrix at least partially surrounding the central area; 
 a fold line formed on the central area to divide the central area into a first side and a second side; and 
 a second line of weakness and a third line of weakness at least partially formed within the central area, the second and third lines of weakness being symmetric about a lateral axis of the substrate and a longitudinal axis of the substrate, the second and third lines of weakness being coupled to the first line of weakness or the fold line, wherein, when the central area is folded about the fold line and the removable portions are removed, the second and third lines of weakness substantially align to create an opening for suspending the form. 
   
     
     
         20 . The apparatus of  claim 19 , wherein the second and third lines of weakness intersect at the fold line. 
     
     
         21 . The apparatus of  claim 19 , wherein the lateral axis is spaced from a first end of the central area by a first distance and from a second end of the central area by a second distance, the first distance being greater than the second distance. 
     
     
         22 . The apparatus of  claim 21 , wherein the first end is opposite the second end. 
     
     
         23 . The apparatus of  claim 19 , wherein the substrate comprises four perimeter die-cut edges. 
     
     
         24 . The apparatus of  claim 19 , wherein a perimeter of the substrate is not coupled to another substrate. 
     
     
         25 . The apparatus of  claim 19 , wherein the longitudinal axis comprises the fold line.
