Abstract
The present invention relates to methods and compositions for providing hematopoietic function in immunodeficient human patients, by selecting an expanded human umbilical cord blood stem/progenitor cell sample without taking into account the HLA-type of the expanded human cord blood stem/progenitor sample or the HLA-type of the patient; and administering the selected expanded human cord blood stem/progenitor cell sample to the patient. Methods for obtaining the expanded human cord blood stem/progenitor cell samples, banks of frozen expanded human cord blood stem/progenitor cell samples, and methods for producing such banks are also provided herein.

1 . A method of supplementing a cord blood transplant in a subject in need thereof comprising:
administering to the subject a cord blood transplant, comprising one or more grafts that match the subject at at least 4/6 typed HLA antigens; and administering a pool of expanded cord blood stem cells comprising a plurality of cord blood stem cell samples derived from different humans at birth and pooled wherein none of the expanded cord blood stem cell samples in the pool have been matched with the HLA-type of the subject; thereby supplementing a cord blood transplant in the subject in need thereof.
2 . The method of claim 1 wherein the pool of expanded cord blood stem cells provides temporary myeloid engraftment.
3 . The method of claim 1 wherein the grafts provide long-term myeloid engraftment.
4 . The method of claim 1 wherein the subject is in need thereof due to acute myeloid leukemia.
5 . The method of claim 1 further comprising selecting the pool of expanded cord blood stem cells from a plurality of pooled, frozen, and stored pooled expanded cord blood stem cells wherein the selecting is without regard to the HLA-type of the sample and without regard to the HLA type of the subject.
6 . The method of claim 1 wherein the administered pool of expanded cord blood stem cells has not been HLA-typed.
7 . A method of providing temporary hematopoietic function to a subject in need thereof comprising:
administering a pool of expanded cord blood stem cells comprising a plurality of cord blood stem cell samples derived from different humans at birth and pooled wherein none of the expanded cord blood stem cell samples in the pool have been matched with the HLA-type of the subject, thereby providing temporary hematopoietic function in the subject in need thereof.
8 . The method of claim 7 wherein the subject is in need thereof due to an intensive chemotherapy regimen, a myeloablative regimen for hematopoietic cell transplantation, or exposure to acute ionizing radiation.

The method of claim 7 further comprising selecting the pool of expanded cord blood stem cells from a plurality of pooled, frozen, and stored pooled expanded cord blood stem cells wherein the selecting is without regard to the HLA-type of the sample and without regard to the HLA-type of the subject.
10 . The method of claim 7 wherein the administered pool of expanded cord blood stem cells has not been HLA-typed.
11 . The method of claim 7 wherein the expanded cord blood stem cells have been expanded with a Notch agonist.
12 . A method of promoting hematopoietic reconstitution in a subject in need thereof, comprising
administering to a subject a cord blood transplant comprising one or more grafts that match the subject at at least 4/6 typed HLA antigens; administering to the subject a pool of expanded cord blood stem cells comprising a plurality of cord blood stem cell samples derived from different humans at birth and pooled; and monitoring the subject's neutrophil count, wherein the hematopoietic reconstitution comprises achieving 100 or more neutrophils per microliter in the subject's blood in a reduced timeframe as compared to a subject administered the cord blood transplant but not administered the pool of expanded cord blood stem cells, thereby promoting hematopoietic reconstitution in the subject in need thereof.
13 . The method of claim 12 wherein the expanded cord blood stem cells have been expanded with a Notch agonist.
14 . The method of claim 12 wherein the reduced timeframe comprises less than 19 days.
15 . The method of claim 12 wherein the hematopoietic reconstitution comprises achieving 500 or more neutrophils per microliter in the subject's blood, and the reduced timeframe comprises less than 26 days.Cited by (0)
