Devices, systems and methods for tissue resection
Abstract
Exemplary embodiments of the present disclosure relate to devices, systems, and methods for tissue resection in a body lumen of a patient, and may include an elongate body having a cavity at a distal end and a tissue retractor extendable distally from the distal end of the elongate body. The tissue retractor may include an expansion mechanism. The expansion mechanism may include a plurality of arms each having a first end coupled around a distal cap and expandable radially outward from the distal cap such that an anchoring mechanism on a second end of the arms is engageable with selected tissue for resection of the body lumen. The tissue resection device may further include a tissue resecting device.

1 . A device for tissue resection in a body lumen, said device comprising:
a body extending along an axis and configured to selectively anchor with respect to a proximal location along a wall of a body lumen proximal to a selected tissue for resection; a cap distal to said body and configured to selectively anchor with respect to a distal location along the wall of the body lumen distal to the selected tissue for resection, said body and said cap coupled together for axial movement relative to each other; a tissue capture device configured to engage tissue selected for resection and to pull the engaged tissue radially inwardly into said device; a tissue closure device configured to close the engaged tissue circumferentially; and a tissue resecting device configured to resect tissue radially inward of the closed tissue.
2 . The device according to claim 1 , wherein said tissue capture device is a mechanical grasping device selected from the group consisting of: axially extending posts, mechanical fasteners, an expandable and contractable ring attachable to tissue, or a balloon with mechanical fasteners attachable to tissue.
3 . The device according to claim 1 , wherein said tissue capture device extends circumferentially around said device.
4 . The device according to claim 1 , wherein said tissue capture device comprises a suction device.
5 . The device according to claim 4 , wherein said suction device extends circumferentially around at least one of said body or said cap.
6 . The device according to claim 1 , wherein said tissue capture device extends circumferentially around the interior of said device.
7 . The device according to claim 1 , wherein said tissue capture device extends circumferentially around the exterior of said device.
8 . The device according to claim 1 , wherein said tissue capture device extends axially to capture tissue.
9 . The device according to claim 1 , wherein said tissue closure device comprises a plurality of staples.
10 . The device according to claim 1 , wherein said tissue resecting device extends axially to resect tissue.
11 . The device according to claim 1 , wherein said tissue resecting device comprises a blade.
12 . The device according to claim 1 , wherein said tissue resecting device extends from said cap to resect tissue.
13 . The device according to claim 1 , wherein said body and said cap are configured for passage of an endoscope therethrough.
14 . A system for tissue resection in a body lumen, said system comprising:
a tissue resection device comprising:
a body extending along an axis and configured to selectively anchor with respect to a proximal location along a wall of a body lumen proximal to a selected tissue for resection;
a cap distal to said body and configured to selectively anchor with respect to a distal location along the wall of the body lumen distal to the selected tissue for resection, said body and said cap coupled together for axial movement relative to each other;
a tissue capture device configured to engage tissue selected for resection and to pull the engaged tissue radially inwardly into said device;
a tissue closure device configured to close the engaged tissue circumferentially; and
a tissue resecting device configured to resect tissue radially inward of the closed tissue; and
a visualization device for visualizing positioning of said tissue resection device in the body lumen relative to the selected tissue for resection.
15 . The system according to claim 14 , wherein said body and said cap are configured for passage of said visualization device therethrough.
16 . The system according to claim 14 , wherein said visualization device is an endoscope.
17 . A method for resecting tissue in a body lumen, said method comprising:
anchoring a proximal section of a tissue resection device with respect to a proximal location along a wall of a body lumen proximal to a selected tissue for resection, and anchoring a distal section of a tissue resection device with respect to a distal location along a wall of a body lumen distal to a selected tissue for resection, thereby demarcating a selected tissue portion site with the proximal section and the distal section of the tissue resection device; drawing tissue along the selected tissue portion site radially inwardly and longitudinally into the proximal section of the tissue resection device; securing together apposed tissue drawn radially inwardly within the proximal section of the tissue resection device; and resecting tissue between the secured-together apposed tissue.
18 . The method according to claim 17 , further comprising:
advancing the proximal section and the distal section of the tissue resection device to the selected tissue for resection; anchoring one of the proximal section or the distal section of the tissue resection device with respect to the selected tissue for resection; moving the other of the proximal section or the distal section of the tissue resection device away from the one of the proximal section and the distal section of the tissue resection device, and then anchoring the other of the proximal section or the distal section of the tissue resection device with respect to the selected tissue for resection.
19 . The method according to claim 17 , further comprising using suction to anchor at least one of the proximal section of the tissue resection device of the distal section of the tissue resection device with respect to the body lumen.
20 . The method according to claim 17 , further comprising reversing anchoring of the proximal section and the distal section of the tissue resection device after resecting the tissue.Cited by (0)
